    You may apply for an extension of credit as a business, individually, or jointly. If you intend to apply for credit jointly with another individual or business, please check the box below and initial.

    This Application may be completed jointly by co-applicants or guarantors. All persons signing this Application for Credit hereby agree that the application for an extension of credit is voluntary; and acknowledge Academy Bank may not require a specific individual to jointly apply, with the exception of owners of the business entity.

    If the Application for credit is given jointly, all persons giving this Application should sign. All persons signing this Application hereby authorize each and every other person who signs this Application to sign amended or supplemental Applications from time to time hereafter on behalf of any and all persons signing this Application until such authority is withdrawn by written notice delivered to said bank. The above-named bank is hereby authorized, from time to time at its discretion, to check the credit and employment history of each person signing and to answer questions about said bank's credit experience with each such person.

    IMPORTANT INFORMATION: The federal government requires all financial institutions to provide the following notice to commercial applicants with gross revenues of one million dollars or less.

    If an application for business credit is denied, the applicant has the right to a written statement of the specific reasons for the denial. To obtain the statement, the applicant should contact their lender or call Academy Bank at 1-888-377-1568 x83508 or write to Academy Bank, 99 NE 72nd St, Gladstone, Missouri 64118, within 60 days from the date the applicant is notified of our decision. A written statement of reasons for the denial will be sent within 30 days of receiving the request.

    Each Owner/Officer of the Business signing below certifies that: (1) the information provided in this Application with respect to the Business (including any attachments) is true, correct and complete in all material respects; (2) the personal information provided in this Application with respect to such Owner/Officer is true and correct; (3) the undersigned are authorized to submit this application on behalf of Business; and (4) Academy Bank is hereby authorized, from time to time at its discretion, to check the credit history of Business and the personal credit and employment history of each person signing this application, and to answer questions about Academy Bank's credit experience with Business and each such person.

    This Application may be executed in one or more counterparts and on separate counterparts; such counterparts and signature pages will be deemed compiled into one fully-executed Application even if not physically attached to each other. A signature delivered by e-mail, facsimile or other electronic transmission will be deemed to constitute an original and fully effective signature.
